As you complete this online registration below, kindly add these necessary details in the "Special Instructions" section of the sign-up:

• Vendor Category (choose one): Disc Jockey, Videographer, Photographer, Photo Favors, Invitations, Florist, Decorations, Party Favors, Formal Wear, Clothing, Other
• Please place me in the same spot as last year if possible (Yes / No)
• Table needed (Yes / No)
• Table cover needed (Yes / No)
• Number of chairs needed
• Electricity needed (Yes / No)
• If electricity is needed, please list all items being plugged in

Payment is accepted via credit card on this registration page or via check delivered to the M'kor Shalom office.

You may also download the Vendor Registration Form and return the completed form to the M'kor Shalom office with your space requests/requirements.

Please Note:

• Registration forms will be accepted only when accompanied by payment in full (online or in the office).
• Vendors and displays are to be confined in the booth space and not to exceed 2-feet from the front of the booth. Aisles must be kept clear.
• The vendor directory will be printed from the information supplied on this form. Please review the information carefully, as there will not be a follow-up mailing. In order to be included in the directory,  your registration and payment MUST be received by December 30, 2018.
• A list of all attendees will be e-mailed to all vendors as an Excel document (containing names, addresses and phone numbers, along with the date and type of event). Be sure to provide your e-mail address.
• Setup on January 13 will begin at 8:00 am and must be completed by 9:45 am. The doors will open to the public promptly at 10:00 am.
